We offer three plans so that you can find one to fit your needs.  The PPO plan allows you to pay co-pays for office visits.  With the popular HRA plan, Trellis will provide you with a credit card that will help with up front claims expenses and with the High Deductible Health Plan you can participate in a Health Care Savings Account.  You can cover children under all three plans. 


Dental through Ameritas:

You receive up to four cleanings a year as preventative along with a rewards program offering savings!

Offers a large provider network with an annual eyewear allowance.

$20,000 paid by Trellis


Supplemental Life Insurance through Unum

Ability to purchase additional life insurance for you or your family.

Payable  after 6 months of becoming disabled.
