In cooperation with the American Lung Association (ALA), Trellis Supportive Care is pleased to offer a specialized Advanced Lung Care program to support the in-home treatment and care of hospice patients living with lung disease. Our Advanced Lung Care program represents the best standards of care for end-stage pulmonary disease, as defined by the ALA.
To qualify for the Advanced Lung Care Program, patients must meet eligibility for Hospice care. With Hospice care, patients and their families can expect advanced treatments and support at home that includes care for fluid overload, respiratory assessments, medications, therapeutic interventions, complete symptom management, and more.
A team of caregivers will support both the patient and family with medical care as well as emotional and spiritual care.
Our hope is that our Advanced Lung Care program improves patients’ quality of life by managing symptoms while providing a consistent and proactive approach to in-home care for those living with advanced pulmonary disease.
