As trusted experts offering the most compassionate and comprehensive care, we are here to guide patients and families through these difficult moments in life’s natural progression.


Our Hospice team will come to you, wherever you call home.

Under the direction of a primary physician, our team develops a comprehensive plan for offering emotional support and comfort to patients and families. Hospice staff are available 24 hours a day, seven days a week — tending to patients in scheduled visits as well as coming to the home when needed for special attention.

We care for people, regardless of diagnosis. Although some hospice patients have cancer as their primary diagnosis, Trellis Supportive Care provides care to patients of all ages who are dealing with any potentially life-limiting illness, including Alzheimer’s, congestive heart failure, COPD, dementia, and emphysema, to name a few.

At a time that may seem hopeless, you don’t have to face it alone. We are here to give you help, hope, and support when you need it most. We will be with you every step of the way.

Complementary Therapies

Complementary therapies add an extra layer of care to nurture the body, mind, and spirit. These non-invasive, holistic practices do not replace the medical, social, or spiritual care of the hospice team. Rather, they work with the total care of the interdisciplinary team to promote comfort and wholeness for both patients and their families. Volunteers provide companionship, a listening ear, an opportunity for a caregiver to take a break, and so much more. Our volunteers represent the cultural diversity of our community and are paired with patients and families that request a volunteer for an added layer of support.

For patients needing more care than can be provided at home, our Hospice Home promises care in a comforting, family-like environment. You'll find 40 private patient rooms offering state-of-the-art care in a beautiful home-like setting. Patients may be admitted when the necessary medical care is not easily managed at home, or when a caregiver is in need of respite.
