GUIDE Program – Support for Dementia Patients & Caregivers
This program is designed to help dementia patients have more quality of life, help family caregivers feel supported, and help dementia patients stay in their homes longer.
For families struggling with dementia, this program may provide relief.
With the growing number of people facing dementia, the Centers for Medicare & Medicaid Services (CMS) are conducting a groundbreaking program called GUIDE, which stands for Guiding and Improved Dementia Experience. Trellis Supportive Care was selected to implement this program locally as part of an 8-year study.
Your Trellis Supportive Care team will provide support and care in keeping with the GUIDE programs focus on comprehensive, coordinated dementia care. The program goals are to improve the quality of life for people with dementia, reduce the strain on their unpaid family caregivers, and prevent or delay long-term nursing home care.
Who Can Participate?
Patients with confirmed dementia diagnosis, including Alzheimer’s, Lewy Body Dementia, Frontotemporal Dementia, vascular dementia, etc., who are living at home, in assisted living, or on a memory care units, and are enrolled in Medicare A and B. Please note that patients are not eligible if they live in a long-term care facility, are enrolled in Medicare Advantage, Special Needs Plans, or participate in Hospice or a PACE organization.
What are the key benefits?

- Collaboration with other providers, including your primary physician
- Respite care/In-home support (up to $2500 or 83 hours per year) for unpaid caregivers
- Personalized education, safety assessments, and Caregiver Skills training
- Care coordination and connection to resources
