Partnerships
Since our very beginning in 1977 when our founders first met, we have been grateful for the many relationships and partnerships built on a shared passion for providing compassionate end-of-life care and support.
Deep roots and community partnerships strengthen the mission
Hospice care, palliative care, grief counseling, and the other programs and services offered by Trellis are easy to embrace. We are thankful for the many hospitals, long-term care facilities, specialty medical practices, senior care organizations, and so many other partners who want to make sure that anyone who can benefit from our care has easy access to the help and support they need.
Likewise, there have been countless businesses and individual donors who have invested in our organization, knowing that ultimately, a gift to Trellis is a gift to the patients and families who will turn to us in their time of need.
We are grateful for the decades of support and faith in our team.
